Jack is the son of Dr. Scott Hayworth and 61-year-old, Dr. Nan Hayworth of Bedford, New York. Dr. Scott Hayworth is a CEO and Market Leader of Optum Tri-State Region, having become its president in March 2021. One of five regions in the Optum nationwide network, Optum Tri-State comprises three large multispecialty groups, CareMount Medical, ProHEALTH NY, and Riverside Medical Group, serving 1.6 million patients at 630 locations. 61-year-old, Dr. Nan Hayworth, is an American ophthalmologist and former Congresswoman for New York’s 19th congressional district. A Republican, she was elected in 2010.

Scott Hayworth, Jack’s father shared the news of his death on his official Facebook page, according to the post he said Jack is a resident of Virginia and worked at the Department of Homeland Security, he also added that he and his wife Nan will miss him. “We, unfortunately, lost our son Jack. A resident of Virginia, Jack worked at the Department of Homeland Security. Nan and I will miss his great sense of humor and wonderful heart. Details to follow.”
